Are you sure your fan is working ? Do you hear it spinning when doing an intensive job ?

On a machine that age

I'd open it, blowing out the dust, and replacing/repairing the thermal paste.

Dust accumulates on the fan blades and settles on the board and parts. Sort of insulating the board and reducing the efficiency of the fan. The thermal paste dries out reducing it's efficiency

BE CAREFUL when you blow out the dust and remove replace the paste... or you could short out the board.

You need to run some diagnostics, first, you should download iStat Pro and check your fan speeds. Second, use Disk Utility (/Applications/Utilities) to verify your startup disk. Please also describe what exactly starts these freezes and what are the symptoms.

Some easy fixes you might want to try would be to reinstall the OS and reseating the RAM.
