when i change the disc when ps4 is on it doesn't read it.but when i turn it off ,it starts reading it but stops after 13 gb.i have to restart it or turn it off then again turn it on inorder to make it read

Could you clarify please? If you turn it off, it shouldn't start doing anything.
If your blu-ray drive isn't reading consistently, you will want to clean the lens. If that doesn't help, you will want to replace the laser assembly. Or the entire drive, but that's more costly in exchange for being slightly easier.
