Whenever I plugin to charge it will charge shows battery full

Whenever I plugin my charger, it will charge the battery and shows battery full; if I unplug the charger the battery level shows but will not turn off then discharges rapidly and if I restart the device it shows low battery. Why it is happening like this? How to overcome?

How old is the tablet ? it may be time to replace the battery if its more than a couple of years old This guide shows how to do it
