@rcb17506 what you are describing sounds like issues with your battery. It can't provide enough power when the phone is needing most of the power.
Either your battery is worn out or has a broken safety chip. Start by replacing the battery and then re-evaluate. Since iFixit does not yet have a specific guide for that use something like this to help you out. Batteries are readily available at many online stores.
While you work on your phone, take lots of pictures. Once you are done with it, take those pictures and create a guide for iFixit.
