You have to remove the rear cover and camera lens cover, unscrew the midframe and remove it, then take out the battery. This will allow you to access the flex cable connecting the screen to the motherboard, which you'll unplug and peel off of the metal frame. At that point you can use heat and/or alcohol to soften the screen adhesive, removing it with tools such as an opening pick and a suction cup. Clean off the old adhesive and assemble the phone far enough to turn it on to ensure the screen works before gluing it down.

Tape down the glass so you won't end up with shards everywhere. Heat the glass with something like an iOpener, and take off the glass with a suction cup. Replace.
