Contact your wireless company and follow their procedure. Other than that there is nothing you can do.
Verzion states that 4G and LTE phones are not locked Here
ATT has a website to send a Request
Sprint has instructions Here
And lastly T-Mobile provides instructions Here
I don't know which carrier you have, but these are the most popular in the US. Otherwise find a support number for your Carrier and they will help you.
