The Dyson Ball Multi Floor Upright Vacuum was released in 2012 and can be identified by the Dyson tag DC40. It is very similar in design and mechanical operation to its earlier counterpart, the DC41 Animal.

Why is only one side of the brush bar turning?

Only one side of the brush bar is rotating when in use.

The brush bar axle has broken. Mine broke also due to a bunch of hair wrapped around axle (daughter has long hair). It is a piece less than 2 inches long, 1/4 inch round. Has a screw on each side allowing for easy replacement. Problem is...I cannot seem to find the brush bar parts sold separately.

This is exactly what is going on. If you take the housing apart like you're going to change the brush out (instructions for that are on the dyson site), there's a little piece broken off inside the brush. I cannot find the axel sold separately either - I had to buy a whole new cleaning head. Still cheaper than a new vacuum though.
