Best to get a new MagSafe power adapter as it looks like the cable got caught on something Vs worn. Do make sure you get a REAL Apple MagSafe adapter! Here's a bit more on the fakes that are out there: Counterfeit Macbook charger teardown: convincing outside but dangerous inside & Lacking safety features, cheap MacBook chargers create big sparks. These chargers can damage your system! Make sure you go to an Apple Store (or online) or an authorized seller of Apple gear.

And yes you are going to need a new battery! Are you up to doing it your self? Here's the IFIXIT replacement battery kit: MacBook Pro Retina (Early 2015 13" & Mid 2015 15") Battery

Well, to start with you likely need a new battery if SMC is telling you the battery needs to be replaced. I would recommend you run coconut Battery to get a better idea on your batteries condition also check your MagSafe adapter & its connectors - Apple Portables: Troubleshooting MagSafe adapters. Make sure the adapter cable and connectors (cable & body) are in good shape. Post a a screenshot of the full main screen of coconutBattery so we can see what it's telling us.

You also appear to be running hot per the TG Pro log. So you do need to do some cleaning of dust build up internally. It would be helpful seeing the a screenshot of the main screen as well.

Thank you @danj it's very helpful.

But what is the causes of suddenly turn off when I open a heavy application like : Photoshop, skeatchUP ... ?

You appear to have multiple issues here.

Lets first get the system working with a good battery & charger.

As I said you also have a heat issue. When you push your system into heavy processing the system heats up and needs to dissipate the heat.

I also need a better view on whats happening than the TG Pro log file. I really need a screenshot of the main screen so I can see the high temp water marks and see if the fan is cooling properly.
