How to fix HMD disconnection?

My Oculus Rift headset is constantly disconnected, and the "We can't find your headset..." notice will pop out, I have tried everything on the software end including setting usb power management and got the officially recommended Inateck PCI-E USB card, which didn't help either. The headset will disconnect randomly and also when I give pressure on the top of the device, especially on the top left nearby the HDMI plug. Wondering if this is caused by the hardware defect and it would be highly appreciated if you could tell me how to fix it. Thank you very much!

It sounds like a defect. Like a loose wire. My friend had one that had the same problem and he just sent it in under warranty, is your's under warranty?
