Did you try reseating the cable that connects the charging port to the motherboard inside the phone? When you replaced the screen, you would have disconnected the charging port cable form the motherboard, so it's possible that it's not fully inserted.

How do i know it is fully inserted?

You just need it to click in and it should be somewhat level and not in an angle. Other than that it's probably a accidentally damaged charging port dock connector assembly during repair.

The one crucial question is that did you charge the phone after screen replacement before it went flat? Did that battery percentage go up at all if you did?

Ben, I have just done the same thing, my charge is going up slowly like 2-3% every 4 hours, does this mean its not seated properly? or Shall i just get a new battery.
