Strange Sound in Headphones

I have the Xenyx 502 mixer. I am trying to set it up. I have an xlr mic going in. I am getting almost no sound in my headphones. When I turn the main mix up past about 40% I get a high-frequency sound. As I continue to turn it up the frequency of the sound gets lower and lower. Any thoughts on what this might be?

Is the mic the only input connected when this happens?

Are the LED lights showing any signal level variations when using the mic? The LEDs are connected in parallel to the input of the headphones amp, so if the LEDs are showing but there is little or no headphones sound then perhaps the headphone amp may be faulty.

If the are no LEDs showing, is the TAPE to MON switch in the 'normal' position?Looking at the circuit, this switch, when operated cuts off the headphones from every other input except the tape input.
