Daniel, mis-aligned lasers, or malfunctioning diodes can cause this.

Hey Daniel, you installed the actually firmware?

I would say, your Blu-Ray lense is faulty. You can clean the lense with special dust-remove discs. Search at eBay.

If you have no success, you have to replace the disc drive.
