I need to replace the 'all in one' cable

The three connections between my MacBook and the display are worn by years of use. What is the part number of the ‘all in one’ cable for this display and how do I replace it?

Is it the DisplayPort version or the Thunderbolt version?

They look exactly the same! Make sure.
