Model number of and place to buy airport card

I have searched and searched, but I cannot find anywhere to buy the Airport Extreme + Bluetooth 2.0 + EDR Combo Card for the late 2005 Power Mac G5. Just a model number for the card would be helpful as well. Thanks!
