Headphone Jack MacBook A1286 replacement

Where can I find a Headphone jack replacement fro a macbook pro non-retina ??

(pretty easy to find for retina but not for the unibody A1286 model...)

It's integrated into the logic board, see step #21 of this guide: MacBook Pro 15" Unibody Mid 2012 Logic Board Replacement

The sound change from headphone to speakers very easily ( I mean I just have to touch a little bit my headphone and it goes on and off back and forth....So, that's why I think it's require a new one...

I would open it up and see if you solder connections to the logic board are good. Gentle see if it will move with your finger and visually inspect it. I'd also try another input device to make sure it is the port.
