I'd recommend the following test to figure out if you have a software or a hardware problem:
Install OS X on an external hard drive. Start up your computer from that external hard drive instead of your internal hard drive. (If you plug in the drive and then press "option" when restarting, you'll be presented with a window that will let you select the drive you're starting up from.)
If the Airport card shows up when you've started up from that external hard drive, it's likely there's a software problem with the internal hard drive. The extreme solution would be to back up your data, then wipe the internal drive and reinstall OS X and your other applications. A less extreme solution might be to install any updates to OS X that you might be missing. Sometimes updates will correct problems with an existing installation.
If the Airport card does not show up when you've started up from the external hard drive, chances are excellent that it's a hardware problem. The first thoughts that come to mind are that you might need to open up the computer and reseat the card -- could be that a connection has come loose. Or the card might need to be replaced.

it may be an issue with the airport card or possibly a power issue with it. try resetting the smc on your mac. most macbooks this is accomplished by removing all cords and the battery and holding your power button down for 5 seconds.
