Headphone ports get "tired". There are spring-loaded, mechanical contacts inside that weaken with use and environmental abuse.
If you can confirm on another phone or device that your headphones are fine, then I would replace the Lightning Port/Headphone jack assembly. Follow this guide.

I've tried it on my younger sister's phone, which is the same as mine, but it doesn't work. I'm starting to think it's something to do with the wires.
