First my Original Screen Shattered and my LCD pell off the Screen so, I change the Screen by myself, I Notice that I Forgot to Disconnect the Battery, My iPhone works fine but the Rear Camera don't Work and I Can't turn On the flashlight in the Control Center when I open the Camera App It appears Black and Sometimes appear a Yellow Warning Icon in the Corner

Sounds like the camera ic or components related to it have blown.
This issue sppears to be common on iphone 5/6/6s screen replacements when the battery is not unplugged while doing screen replacements.
When ever the camera app shows black screen with yellow warning icon on top right which indicate camera or phone too hot I've never heard of replacing rear camera to solve that issue so I think its best you take it to a professional that does board level iPhone repairs.
You are lucky you didn't blow a backlight filter when removing or plugging in the screen with the battery on. If it did happen the phone would show display but will be very dim or require a flashlight directly on the screen to see it.
The camera stopped working after the screen replacement not before it from what you said.
