The screen assembly is sold as an LCD + Digitizer, It is quite a bit difficult to do this repair but it can be done.
This guide shows you how to disassemble the phone to take the screen out:

Check the model number which is at the back of the phone at the bottom in text. Buy the screen that is supported with that model number.
Your one is probably SM-G920i or something like that.
