I would reset the PMU and PRAM. Sometime this can help.

If not, it could be that the cable connecting the drive to the board is bad, or coming slightly unplugged, so it may be worth opening the machine to check.

It could also be a bad optical drive, but hopefully one of the above two steps will prove it's not.

Also, and I don't know why this is the case, but I've had drives in that situation with a CD in them and where the drive did not show up to the computer, but once I managed to get the disc out, the drive then showed up and worked again, i.e. somehow with the disc in the drive the drive was in a hung state, and with the disc out, the problem was corrected. So if the above doesn't work, you might want to take steps to remove the disc and see if that helps.

Have you replaced the cable? That would be my next step, and replacing the drive itself would be my next after that. Unfortunately there aren't too many other options.

Is this a new Optical drive? have you installed any new hardware lately? Does your Hard drive have a Jumper Pin? I ran into a situation once where a new hard drive (western digital I believe) had a jumper pin installed, while the pin was in, its cut off the optical drive, removed the jumper, all was well.
