To remove the screen, first you need to work your way around the edges of the phone slowly peeling back the broken glass. There is a slight gap between the plastic frame and the screen glass that you can work your pry tool into. Be extra careful near the top right corner, as that is where the flex cable for the screen glass touch sensors is located. When the glass is completely separated from the phone you should be able to swing the whole thing back and see the LCD underneath (but not too far, remember to be careful with the flex cable!).

Then, flip over the phone. You must detach the flat flex cable at the top (that will be where the new screen glass attaches), and the flex cable on the right hand side (when the phone is face down looking at the back).

Once the two flex cables are detached, loosen the two plastic clips holding the circuit board in near the top of the phone, on the left and the right sides. This will allow you to push the cable for the screen back out through the top of the phone and slide the new one back in place.
