i replaced screen, battery and clean the mother board, the phone was water damaged and after doing these fixes the screen came on and after trying to restore it on itunes i checked teh phone again and it was black i have tried to to the lock and home button restart but does not work

Check your work, make sure the LCD Display connector is perfectly attached to the logic board. If so, then the water damage has effected your logic board and there is no easy fix, of the new screen is faulty and you need to intall another iPhone 4S front screen, digitizer, LCD display, and mounting frame.

Before you do that you may want to know about the black screen of death. Seen this on iPhone 4, 4s & 5.
Do a hard reset. press power and home button for roughly 30 seconds. this reboots the phone. You should only release when you see an apple logo.
If this doesn't work it's going to be a new screen as above

The original poster already tried Reset and it failed. Next step is to be certain that the screen connectors are correctly in place.
