headphone port maybe damaged, So the device does not know when you plug head phone's in.
Before you start opening the device, Do a restart and a restore to see if this will fix the issue.
If you plan on opening up your device, First thing to know is the headphone port is soldered to the main bored.

Also, try and clean your headphone jack. you can use a safety pin or toothpick and see if you have any debris in there. Sometimes a can of compressed air will work, if you hold your iPod upside down and blow a puff of air in there. You also might want to try to insert and remove the headphones a few times in rapid succession, it is possible that the contacts are stuck.
