I just fixed this by replacing the rear glass and camera lens as one piece. It was quite easy about $15 on Amazon and took about 5 minutes using a heat-gun and a plastic card. Be careful to not damage the flex that goes to the daughter board when doing so. Also only allow the card to be 1CM inside the phone when slicing the old adhesive to remove the rear glass.
