Now that you have unscrewed the 2 black screws, the silver plate holding the device to the motherboard should come right off.

Next, gently pull the Audio Output device off of the LCD touchscreen. It is secured by some weak adhesive and should come off easily.

You are now free to install a new Headphone Jack/Audio Output device.
