After power button repair: Proximity , vibrator & headphone jack dead

After successfully repairing my home button last week, I decided to try again to fix my power button. For months, the power button hasn't clicked and i would just have to press the button very hard for it to lock/unlock.
After ordering the new power & sensor cable, I followed the guide to replace it. This repair seemed a lot more difficult than the home button one. I switched over the metal bracket around the power button and I think I also switched over the two sensors with foam on top for the proximity sensor (it's there now).
After trying to put everything back together, I realized I lost a screw :( I think it's the 1.4 mm screw that goes on the top of the logic board.
Upon turning on the phone, I was excited to see that the power button now clicks perfectly, and nothing else seemed to have broken... until I answered a call and realized the proximity sensor wasn't working.
I also turned my phone to vibrate and realized it no longer vibrates.
I then tried checking other aspects of the phone and realized the headphone jack doesn't work. Music will continue to play out of the speakers when headphones are in.
Needless to say, I need to fix this. I've gone into the phone twice more now to try to see if I missed something or if something wasn't put back together correctly. It all looks right to me. Nothing is blocking the vibration motor and it spins fine. It seems that it isn't getting any signal to vibrate from the phone.
Any help would be great! Could it be a bad power & sensor cable (direct from iFixit)... or could it be something else?
