How do I take a parent tap off iPhone5?

Someone has put a parent trap on my phone and I cant get it off, I was wondering if there is anyway possible to get this off? Its a very big inconvenience and hate that an ex can see everything that is going to my phone and calls me about it all the time. So if someone knows a way please let me know. Thank you.

Take it in to your service provider and they will be able to determine your age and remove it if you're the one paying the bill ;-)
